In this vital role, you will serve as a cornerstone in the journey to enhance the lives of our residents and patients, enabling them to achieve optimal functionality and independence in their daily lives. This is an opportunity to make a meaningful contribution by enabling individuals to regain their independence and improve their quality of life.
Responsibilities:
Develop and implement goal-oriented treatment plans tailored to each resident's needs, focusing on enhancing daily living skills, motor functions, coordination, and cognitive abilities.
Provide hands-on therapy sessions that may include exercises, activities, adaptive techniques, and assistive devices to improve residents' functional abilities.
Collaborate closely with the interdisciplinary team, including physicians, nurses, social workers, and other therapists, to ensure comprehensive and coordinated care.
Monitor and document residents' progress, adjusting treatment plans as necessary to achieve optimal outcomes.
Educate residents, families, and caregivers on adaptive strategies and techniques to promote continued progress and independence.
Contribute to the development of discharge plans and recommendations for ongoing care or home modifications.
Adhere to ethical and professional standards while maintaining a caring and supportive demeanor towards residents and their families.
Qualifications:
A valid license as an Occupational Therapist in the State of Texas.
Bachelor's or Master's degree in Occupational Therapy from an accredited program.
Experience working with geriatric populations, preferably in a nursing home or post-hospital rehabilitation setting.
Strong interpersonal skills and the ability to communicate effectively with residents, families, and the interdisciplinary team.
Knowledge of relevant therapy techniques, equipment, and assistive devices.
Ability to assess, plan, implement, and evaluate individualized treatment plans.
Excellent organizational and documentation skills.
Current CPR certification.
